STUDENTS

1. Students who have received at least one (1) dose of each required immunization may be issued a Provisional Certificate effective for 120 calendar days. The students must complete their immunization before the Provisional Certificate expires.

2. Transfer students from out of state may be issued a Provisional Certificate effective for 120 days during which time they are to obtain their immunization records.

3. Students transferring from another school within the district or in Iowa may be issued a Provisional Certificate until their records arrive from the sending school. If immunization records do not arrive with student's other school records, the student must be excluded from attending classes until a Certificate of Immunization is presented to school personnel.

A Provisional Certificate CANNOT be issued for this student.
