STUDENTS

Requirements For a Certificate of Exemption
  1. Medical Exemption

    A blue Medical Exemption card must be signed by a doctor  (medical or osteopathic) and the parents and presented to school personnel before the student can be allowed to attend classes.
     
  2. Religious Exemption

    A blue Religious Exemption card must be signed by the parent, notarized by a notary public and presented to school personnel before the student can be allowed to attend classes.  The religious denomination must be stated on the card.
