Dental Screening Information PARENTS
OF ALL NEW/FIRST TIME 9TH GRADE STUDENTS: The
State of If
you have not provided this certificate, volunteer dental hygienists will
be at our school on January
25, 2012
to
provide free screenings that will meet this requirement. School
dental screenings do not replace regular dental checkups at a dental
office. Please make sure your dental visits are up to date.
If
you would like to have your child screened Jan 25, you do not need to do
anything.
The screening will be completed and a note with the results will be sent
home to you. If
you do not want
to have your child screened that day, please contact the school
nurse prior to January
16, 2012. If
you decline the free screening, you will need to make sure that your
child receives a dental screening outside of school and you will need
to send a certificate as proof of this screening by January 16, 2012. Thank
